The 26-year-old Leeton man allegedly entered a medical centre on Victoria Square in Narrandera about 3pm on Tuesday and demanded prohibited drugs from staff, who refused.
According to police, he then became verbally abusive and threatened the employees before leaving and getting into a car nearby.
Police say he demanded the driver, a woman aged in her 50s, take him towards a Murrumbidgee River boat ramp on Old Narrandera Road.
Once there, the woman was sexually assaulted by the carjacker, police allege.

Passers-by came to the woman's rescue a short time later, after she alerted them.
The woman has since been treated at Wagga Base Hospital.
Her alleged attacker fled in the car, smashing through a gate into private property and parking the car near the river.
When police arrived and attempted to remove him from the vehicle, the man allegedly spat at two constables.
He was eventually arrested and taken to Narrandera Police Station and charged with a string of offences.
Police say the alleged spree resulted in multiple crime scenes, which have been examined by forensic specialists.
He was refused bail and will appear at Wagga Local Court today to answer charges of demanding property with menaces with intent to steal, stalk or intimidate and intend fear of physical harm, unlawfully take/drive motor vehicle with person in it and aggravated sexual assault.
The man was also charged twice with assaulting police and single counts of destroy or damaging property, being an unaccompanied learner driver and not displaying L plates.
