Gerald Louttit heard a knock on the wall of his home on Mair Street, Turvey Park about 11.20pm on Sunday.
He opened the front door and was confronted by a man who punched him in the face and then fled on foot.
Mr Louttit described the attack as "bloody terrifying". He said he turned the verandah light on after hearing the noise, but a section was not covered by the light properly.
"He was sort of backed into the wall," Mr Louttit said.
"I just opened the front door automatically. I said, 'who are you'. The only word he said was 'jack', and he swung a punch, and I ended up across the room."
Riverina police crime manager Adrian Telfer said the man was not known to the victim, and detectives are considering it an "unprovoked" attack.
"We are still trying to learn why this occurred," he said.
"But what we do believe is there was no provocation from the 86-year-old."
NSW Ambulance Paramedics were called and treated Mr Louttit at the scene for a suspected fractured nose before he was taken to Wagga Base Hospital, where he is being monitored for a bleed on the brain.
IN OTHER NEWS:
"It's quite disturbing," Inspector Telfer said.
"They are at their own private residence.
"They have received a knock on the door and he has been assaulted for no good reason."
Mr Louttit said he never expected anything like this to happen to him, especially in Turvey Park.
The couple has lived there for over two years, and Mr Louttit said it was a "good, friendly" neighbourhood.
Officers attached to Riverina Police District were notified and have commenced an investigation.
As investigations continue, police are appealing for information to help locate a man who may be able to assist with inquiries.
He is described as Caucasian in appearance, between 40 to 45 years old, with a slim build, medium-length scruffy hair and a beard.
At the time, he was wearing a denim jacket, black pants and joggers.
Mr Louttit urged anyone with the slightest bit of information to contact the police.
His message was echoed by his wife of 46 years, Robyn, who was home at the time of the attack.
"Anything at all can help," she said. "Even if it turns out to not be anything, still come forward."
Mrs Louttit also encouraged other residents to be aware and only open the front door when they know who is on the other side.
"It happened so quickly," she said. "You can't be careful enough."
Mrs Louttit said they had been overwhelmed by messages of support and kindness since Sunday night, with one neighbour even staying the night, so she was not alone after the assault.
"We are very grateful for the support," she said.
Anyone with information about the incident is urged to call Wagga Police on 6922 2599 or Crime Stoppers on 1800 333 000.
