The defending champions were down 10-4 at halftime but made a smaller and younger Mater Dei outfit pay for costly mistakes and penalties down their own end in the second half.
Kooringal scored first through five-eighth Reilly Waugh-Smith in the early stages, before Mater Dei hit back soon after with a try to captain and hooker Will Hurst to level scores at 4-all.
Kooringal looked poised to retake the lead but Mater Dei centre Harry Russell-Wichman plucked an intercept on his own line before running the length of the field to score under the posts and give his side a 10-4 halftime lead.
However, Mater Dei made life difficult for themselves after the break with errors and poor discipline, and Kooringal made them pay.
Tries to Mitch Carter and Toby Rodaway gave them a 16-10 lead, before a try to skipper and Rick Keast medallist Joel Tracey sealed the victory.
Russell-Wichman scored a consolation try after the siren for Mater Dei, but Kooringal capped a dominant season.
Tracey was man of the match after a huge game, including a big second half hit which lifted his team.
"I think the captain has to make an impact and I that's a good way to do it, putting a big shot on," Tracey said.
"I'm very proud of the team and everyone involved in the school."
Tracey said the team didn't panic at half time.
"We just stayed composed and relaxed, we knew we had the team to do it," he said.
"We did it last year and throughout the tournament, we didn't really start well in a lot of games. But the team showed with their efforts how badly they wanted to go back-to-back."
Last week's rain meant the semi finals were played just before the final, with Mater Dei holding off Kildare 10-8 and Kooringal accounting for The Riverina Anglican College 30-4.

Mater Dei skipper Will Hurst said the focus was on completing sets in the second half to tire out the bigger Kooringal side, but errors let them down.
"Errors came into our game and we gave them a lot of chances, which they capitalised on," he said.
"Our game plan in the second half was to hold onto the ball and try and tire them out, because they're the bigger pack, but our mistakes didn't let us do it.
"We didn't have many year 12s and we even had year nines in the team, so in coming years we'll be a force to be reckoned with."
