The horror two-car crash left a 44-year-old Mount Austin man dead with two more men taken to hospital on Tuesday.
Police confirmed two vehicles collided on the road near Downside, around 15 kilometres north of Wagga between the Olympic Highway and Downside Village Road, just before 5am.
A spokesperson for the Murrumbidgee Health District confirmed on Wednesday that a 61-year-old man had been transferred to St George Hospital, in a serious but stable condition.
The 26-year-old man remains at Wagga Base Hospital in a stable condition.
While specific details of how the crash occurred are yet to be determined, Coolamon resident Wayne Colgan said immediate safety upgrades need to be completed.
Mr Colgan said he uses the road at least four times a week.
"There is a lack of passing lanes, some of the corners are tight and the quality of the road in some spots needs to be upgraded," he said.
"Especially in the afternoon, you get more people passing and taking risks.
"Numerous times I have seen people go over the double lines when its unsafe."
Coolamon mayor John Seymour said the crash was a tragedy, especially being so close to home for those in the region.
Cr Seymour said there needed to be discussions between the two councils about improving the safety of the road for all of its users.
Cr Seymour added that it's not the first incident that's happened on the stretch of road between Coolamon and Wagga.

"The amount of traffic that's on that road day and night with people travelling backwards and forwards for work has increased," Cr Seymour said.
"Officially there has been no discussions, but I don't think it would hurt to have a sit down between Wagga and Coolamon councils to develop an idea."
Cr Seymour said there are a couple of potential fixes that come to mind, but official steps would need to be taken to identify the issues.
"I think it needs some passing lanes on it ... I think the bridge at Houghlahans Creek is too narrow, but that all comes under the spectrum," he said.
Wagga mayor Greg Conkey echoed Cr Seymour's comments, adding that Coolamon has grown in popularity as it offers lifestyle options his city cannot.
"As such, it is a popular road, especially in the early mornings and late afternoon when people travel to Wagga from work," he said.
"We would be happy to sit down and listen to any ideas Coolamon had and maybe help lobby for funding if needed. I will be having a talk with our engineers to see if they have any input into it."
Cr Conkey said Wagga and Coolamon council have always worked well together and this was another issue they could join forces on.
He also added that Wagga City Council spent money on upgrading part of Coolamon Road a couple of years ago.
