It is alleged a Phantoms player came off the bench and entered the field of play.
Investigating officers have been employed to see if the player, who is yet to be named, will face any sanctions under Rugby Australia's code of conduct.
Any potential hearing is expected to be held next week.
Southern Inland rugby manager Jack Heffernan would not divulge the player's name while the investigation is ongoing.
"My understanding is Leeton is handling it quite heavily internally as well, which is great to hear, but we have to go through due process also," Heffernan said.

However Heffernan praised the reaction of referee Tim Allen and Leeton officials for their handling of the incident, with the player removed from the ground.
"It is not a good look and we want to stamp that sort of thing out," Heffernan said.
Leeton president John Hill confirmed the player has been dealt with by the club.
However he won't release how.
"According to our code of conduct we can't comment on any cases currently under investigation," Hill said.
"It is being investigated by Southern Inland, they've appointed an investigating officer so will wait for the outcome of that. Any sanctions that we put on the player will remain between the player and the club."
