The form, which is non compulsory, requests people outline the venue they're attending on a given date, contact details and to confirm they haven't been in Victoria in the last 14 days, attended any of the locations on the 'self isolate and get tested' list on the NSW Health Website, have had close contact with someone known to have coronavirus, or have any cold or flu symptoms.
The form can also be completed at the ground, but football operations coordinator Tom Walker is encouraging people to do so beforehand.
"We're asking everyone participating or attending to register using the QR code link either before entering the venue, or at the venue. We're hoping people can do this before entering the venue at any game this season."
Only one submission is required per day for every household.
