The young guns were chased by several Ovens and Murray and Goulburn Valley league clubs before agreeing to terms with the Tigers this week.
Jacob, who stands at 193-centimetres, kicked 66 majors to win the Riverina league goalkicking this year and is set to play at full-forward.
Lucas is 191-centimetres, booted 29 goals and can play at either end of the ground.
The pair are 20 and 17 respectively and will relocate to the Border in the New Year.
Jacob impressed Albury when he turned in a strong performance for Canberra Demons in a practice match at Wodonga Senior Secondary College in 2017.
The pair also have ties with former Tiger premiership ruckman Ken Howe.
Albury co-coach Luke Daly said the Tigers had kept close tabs on the youngsters throughout the season.
"We identified we lacked a bit of height in some areas and saw those two boys as a good opportunity to come in," he said.
"I have a mate who highlighted them to us, we watched a few tapes and from there we went and had a chat.
"Hopefully they fit in well, stick around and become long-time players for the footy club because they are two really good, polite young fellas and that's everything we are looking for."
The Conlan boys' departure continues a bad week for Griffith where they have lost Jordan Iudica (Queanbeyan) and Guy Orton (Coleambally).
The Swans are yet to announce a coach.
