The Hoppers entered Saturday's final round needing results to go their way to finish on top but they did and they brought it up with a 16.14 (110) to 4.7 (31) victory over Leeton-Whitton at Kindra Park.
Coolamon co-coach Jamie Maddox was rapt to jump into top spot and grab the minor premiership.
"It feels really good. It's a good reward for the boys and the work and effort that everyone has put in through the year," Maddox said.
"It's a credit to everyone involved."
Maddox said the first week of finals off would be appreciated at Coolamon.
"It will be handy, don't get me wrong. Especially because we haven't had the bye in a little while so in terms of where we're at, it will be nice for us," he said.
Coolamon dominated from the outset, only allowing the Crows to kick one goal in the first half on the way to opening up a 49-point lead at the main break.
"We played some good footy," Maddox said.
"We got back to our team-orientated, work for each other, footy. It was nice to get a good win."
Luke Redfern was a dominant best-on-ground for Coolamon, with Max Hillier and Jake Barrett also playing strong support acts.
Joe Redfern continued his strong season with a five-goal haul.
The Hoppers chose to rest Jeremiah Maslin, Nick Pleming, Zach Bradley and Ryan Allen for the final round, thinking they were most likely to be playing in the qualifying final.
They will now play the winner of Wagga Tigers and Griffith in the second semi-final.
Matt Rainbird was a clear best for the Crows, who finished with the wooden spoon for the first time since 2007.
