Riverina Police District Inspector Adrian Telfer said about 1pm yesterday, emergency services were called to Cameron Street following reports of a serious assault.
"Police arrived on scene and found a male [a 17-year-old] to be unconscious," he said.
"He was treated at the scene and taken by ambulance to Wagga Base Hospital where he was treated and he was later delivered to Canberra Hospital by helicopter for specialist treatment."
Inspector Telfer said he believes the victim could potentially be suffering from a fractured skull.
A 21-year-old male from Ashmont was arrested and charged shortly after the assault with grievous bodily harm, but police do not believe a weapon was used.
"We believe this was just a physical fight, but the injuries are quite serious," Inspector Telfer said.
"This gentleman fell over and hit his head on the side of the road."
Inspector Telfer said the two men involved in the incident are cousins.
"They have a long history," he said.
"There has been a conflict for some time."
Inspector Telfer said any fight is a concern to police, but the young ages and the serious nature of the injuries were cause for particular concern.
"We believe it was the just two involved," he said.
"We are fairly satisfied that we have spoken to anyone who was there at the time. Police were able to obtain some footage."
Inspector Telfer said it does not appear drugs or alcohol were involved in the matter and the alleged assailant did not suffer any injuries.
