Increasing the efficiency and effectiveness of medical services for patients, particularly those at risk of poor health outcomes; and improving coordination of care to ensure patients receive the right care in the right place at the right time.
Primary Health Networks have been set seven key priorities for targeted work; mental health, Aboriginal and Torres Strait Islander health, population health, health workforce, digital health, aged care, and alcohol and other drugs. The MPHN is working with health service providers, consumers and communities to improve coordination of care, ensuring patients receive the right care in the right place at the right time.
The MPHN is informed by our local health professionals and communities through four regionally-based Clinical Councils, and 33 Local Health Advisory Committees informing a single Community Advisory Council. Regional service centres have been established in each of the four sectors; Riverina, Western, Wagga and Border.
