Mrs Clarke-Lewis died in April 2009 following complications from botched surgery to remove a suspected ectopic pregnancy.
An inquest from deputy state coroner ultimately found surgeon Dr Samy Nassief responsible for her death in 2011.
A plaque and bench seat honouring Mrs Clarke-Lewis' life will be unveiled at Tuesday's ceremony.
Her mother, Narrandera mayor Jenny Clarke, said she would never be forgotten by the community.
"She certainly does live on in everyone's memory because she was so loved by everybody," she said.
"She endeared herself to everybody that she met."
Mrs Clarke-Lewis served as Urana Shire Council's road safety officer for more than four years, from December 2003 to January 2008, and was instrumental in securing numerous government grants for the shire for projects such as a walking and cycling track and tree planting.
Councillor Clarke said her daughter was never far from her mind, more than five years after her death.
"Even though she's been gone five years, there's not a day we don't think of her," she said.
"It was such a tragedy at the time and it still is, it still lives on - it's very hard to get over."
Around 70 people are expected to attend Tuesday's ceremony.
